On 01/27/2023 at 9:17 a.m., Licensing Program Analyst (LPA), Rajani Goudreau arrived at the facility to conduct a pre-licensing inspection. Upon arrival LPA, met with Associate Vice President of Operations, Rodney Carey and Area Director, Lisa Grygera and proceeded to tour the facility. The facility is requested a total capacity of 60 children; including 20 preschool children (ages 2 through 5 years old) in classroom #3 and 40 toddler children (ages 18 through 36 months) in classrooms: 1A, 1B, 2A and 2B. Facility plans to operate Monday through Friday from 7:30 a.m. to 5:00 p.m.

The classrooms were inspected and measured during this inspection and measured a total of 2,413.52 square feet, which is sufficient to accommodate 68 children at one time. The total indoor space for the toddler classrooms measured a total of 1,607.58 square feet, which is sufficient to accommodate 45 toddler children. In addition, the preschool classroom measured a total of 805.94, which is sufficient to accommodate 23 preschool children. Toddler classrooms 1A,1B, 2A and 2B are separated by four feet or higher wall dividers. Toddler classrooms 1A and 1B will have exclusive use of one restroom located between the two classrooms containing one sink and two toilets. In addition, there is one changing table within arm’s reach of a sink between classrooms 1A and 1B. Furthermore, Toddler classrooms 2A and 2B will have exclusive use of one restroom located between the two classrooms containing one sink and two toilets. In addition, there is one changing table within arm’s reach of a sink between classrooms 2A and 2B. Preschool classroom #3 has one sink in the classroom and one restroom with exclusive use containing one sink and two toilets for the children’s use. The number of sinks and toilets is sufficient to accommodate the toddler and preschool capacity's. The restrooms and hand washing areas were observed to be in a safe, sanitary and operating condition, per observation. The classrooms were observed to be equipped with age appropriate furniture and play equipment; including napping equipment (cots), tables, chairs, cubbies, bookshelves, and other materials for the children use. The classrooms are orderly, free of hazards and have adequate heating, lighting and ventilation. There is a kitchen on the premises of the property containing one fridge and three sinks.

The facility will provide Breakfast, lunch, AM and PM snacks to the children. Uncontaminated drinking water is readily available during indoor and outdoor activities through the use of refillable water jugs and disposable cups provided by the facility or brought from home. The staff restroom located in the director’s office and the director’s office will be designated for ill children, if needed. There is one or more functioning carbon monoxide detectors and a first aid kit with required supplies located in the facility, per observation. Disinfectants, cleaning solutions, and other hazardous substances shall be kept inaccessible to the children in care. The facility will be utilizing manual sign in and sign out. Required documents were observed to be posted.

The outdoor play areas were inspected and measured a total of 11,585.76 square feet which is sufficient to accommodate 154 children. The toddler outdoor play area is separated by a four feet high fence. The toddler and preschool children will have separated outdoor spaces. The toddler outdoor play area measured a total of 2,506.80 square feet, which is sufficient to accommodate 33 children. Due to insufficient outdoor space to accommodate the toddler requested capacity of 40 children. Applicant representative has agreed to lower the requested toddler capacity to 33 toddlers. The outdoor space for the preschool children measured a total of 9,078.96 square feet, which is sufficient to accommodate 121 children. The outdoor play areas are fully fenced and includes outdoor play equipment that is age appropriate. There is a play structure located in the preschool outdoor play area that is suitable for ages 2 through 5 years old, per equipment sticker located on structure. There is enough turf material for cushioning under and around the play structure to absorb falls, per observation. There is no play structure in the toddler outdoor play area; however, there is age appropriate play materials and equipment for the children’s use. LPA observed adequate shade in the outdoor play areas by the use of a canopy and building awnings. LPA observed the outdoor activity space surfaces to be free of hazards. There is no evidence of rodent or insect activity, per observation. Proper supervision and placement of staff during outdoor activities was advised to ensure the supervision and the safety of children during outdoor play. There are no bodies of water observed on the premises of the property. In addition, applicant representative confirmed there are no bodies of water on the premises of the property.



LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

This facility plans to provide Incidental Medical Services – IMS. IMS plan on file and approved. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. LPA informed applicant initial water lead testing results shall be received and posted within 180 days of licensure. LPA provided and discussed the following with the applicant: PIN’s 21-04-CCP, 22-06-CCP and 21-21-CCP, External Water Sampler Self-Certification Form (LIC 9275), Child Care Center Sampling Checklist Form (LIC 9276).

A fire clearance for 60 children; including 20 preschool children; ages 2 through 5 years old in classroom #3 and 40 toddler children; ages 18 months through 36 months in classrooms: 1A,1B,2A,2B was granted on 11/15/2023. However, due to the lowered requested capacity from 60 to 53 children an updated fire clearance will be requested.

The following is pending with a due date of 01/30/23:
  • Updated LIC200A reflecting the toddler requested capacity as 33 toddlers, preschool 20 children and total requested capacity as 53 children.

An exit interview was conducted with Area Director, Lisa Grygera. The following reports were discussed and provided: LIC809 pages and Appeal Rights (LIC9058). After final file review and once pending document is received, a preschool license will be issued for 53 children; including 20 preschool children (ages 2 through 5 years old) in classroom #3 and 33 toddler children (ages 18 through 36 months) in classrooms: 1A, 1B, 2A and 2B. Once the license is issued the license will be mailed to the applicant and once received the license shall be posted in a prominent location in the facility.
