- Make all sinks and toilets in the children restroom children appropriate.
- Obtain scissors and thermometer to have a complete first aid kit.
LPAs toured the facility inside and outside per facility to ensure all corrections were met. During time of inspection, LPAs observed that all corrections were met with the exception of placing the age verification sticker on the playground structure, placing childlike stools for the two sinks in the bathroom, obtaining and installing secured gates at the bottom and top of the stairs located near the bathroom. In addition, LPAs observed a door that is located right of the children's bathroom door, to be a direct exit which leads to a busy street. Due to fire code allowing no hindrance of the exit, Ms. White stated she will purchase a door alarm for this door that leads to the street and main entry door to ensure staff are aware once the door is open. In addition to these alarms, LPAs reminded direct visual supervision shall be maintained at all times to ensure the health and safety of all children. LPAs took photos and videos during inspection. See file for copy.
A license for 30 preschool children (ages 2-5) may be granted upon receiving the above corrections and after conducting a final file review. Ms. White stated she will provide proof of remaining corrections to LPA Clenista via photos and videos no later than 02/22/24. Ms. White stated she understands that this report shall be kept available at the facility for public review for 3 years.
Due to computer error at conclusion of visit, this report was drafted in the system when LPA Clenista returned to the office. Copy of this report will be emailed to the Ms. White and Ms. White understands that she is required to respond to the email confirming receipt within 24 hours (which will act as her signature). |
