On 06/21/2024 at 11:55am, Licensing Program Analysts (LPAs), Selina Siao and Michelle Hood conducted an unannounced Case Management inspection for an increase in capacity by adding an infant and school age component to the facility. Upon arrival, LPAs met with staff Michelle Woolfolk and did a headcount of the children in care. Present during today’s inspection were 11 children with Director Chastity Harrell and staff Mekhi Thompson. An ABA therapist was also at the facility in the preschool classroom. LPAs also met staff Mymiona Johnson who contacted the licensee Vera White during the inspection for her to be at the facility. Appropriate ratios and supervision were observed. Facility is currently licensed to care for 30 preschoolers (ages 2 to 5 years old) in classrooms B and C. Applicant has requested to be licensed for 6 infants (ages 12 to 24 months) in room A, 24 preschoolers (ages 2 to 5 years old) in rooms B and C and 9 school age children (ages Tk to 13 years old) in room D. Facility uses removable partitions to divide the three different components. Hours of operation is Monday to Friday from 6am to 6pm. All indoor and outdoor activity space utilized for the children were inspected today. LPA Siao informed licensee Vera White that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities.

LPAs continued to tour the facility and measured all the indoor and outdoor areas due to the changes of the facility. Total preschool areas classroom B & C (one large area) measured to be 852.60 square feet, which is sufficient to accommodate the requested capacity of 24 children. The school age classroom D measured to be 328.05 square feet, which is sufficient to accommodate the requested capacity of 9 children. The infant room A measured to be 286.31 square feet, which is sufficient to accommodate the requested capacity of 6 older infants as the room does not have a crib area. LPAs observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Drinking water is available inside and outside the classrooms via the children's water bottles. Licensee stated that the children's water bottles gets refill by the three-gallon water that the facility purchase. LPA observed all hazardous items to be inaccessible to children. Licensee stated that there are no bodies of water or weapons on the property. Fire clearance was granted by the fire department on 06/06/2024.

LPAs observed a total of 3 sinks and 2 toilets available for the preschool children’s located in the (women's restroom). The school age children will be using the two toilets, 1 urinal and 1 sink located in the (men's restroom). There is a sink and toilet inside the men's toilet that is designated for ill children to use. These are sufficient to accommodate the preschool and school age children capacity. Licensee will be purchasing a portable sink and changing table to be in the infant classroom A.

The facility currently has a fully fenced playground area. There is a chain link fencing that is at least four feet high. The total square footage for the preschool outdoor activity space is 2638.49 square feet, which is sufficient to accommodate the requested capacity of 24 preschooler. Applicant would like the school age children to share the preschool playground with the school age children. The total square footage for the infant outdoor activity space is 248 square feet and it is only sufficient for 3 children at one time, insufficient for the number of children being requested. A playground waiver to share the playground at different times in the AM and PM due to limited square footage shall be submitted and approved prior to licensure. There is a shade canopy structure located at the preschool playground. Licensee will be using a canopy shade for the infant play area. There are sufficient outdoor age-appropriate toys and play equipment available on the preschool playground. There is a climbing structure on the playground for children ages 2- to 12-year-old which is properly anchored. There is adequate cushioning in fall zones of climber provided by rubber chips for the preschool playground. The infant playground does not have any play structure or toys during today's inspection. LPAs did not observe any hazardous items on the playground. There is an outdoor shade area with bench tables and chairs that will be used for school age children to eat. Applicant Vera White was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

The following corrections are needed prior to the issuance of the license:
  • There shall be a sink and a changing table next to the sink in the infant room. Applicant shall provide the dimensions of the items to LPA and submit pictures.
  • Signs should be place for the two school age toilets and sink to indicate that it is for school age children to use.
  • There shall be a sign posted for the isolation sink and toilet that will be used for all three components and a stepping stool is needed in the isolation toilet and sink.
  • There shall be a gate place at the entrance of the preschool classroom B to ensure children's safety.
  • The infant play area shall have age-appropriate toys and equipment available.
  • Adequate shade is needed for the infant outdoor area.
  • Infant teacher qualifications and responsibilities shall be submitted.
  • School age teacher qualifications and responsibilities shall be submitted.
  • Outdoor Activity Space waiver request is needed for the school age children to share the outdoor activity space with the preschooler by schedule per 101538.2 (b) as the school age children does not have their own outdoor play space.
  • Outdoor Activity Space for Infants waiver request is needed for the Infant Outdoor area as the outdoor area is only sufficient for 3 infant and not sufficient for the requested capacity of 6 infants per 101438.2(a).
  • The separation wall for the infant program shall be sound absorbent and be at least four feet high.

Applicant understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied.
