On 01/07/2025 at 12:50 pm, Licensing Program Analyst (LPA) Michelle Hood conducted an unannounced complaint inspection regarding the above allegations, LPA met with the Director. LPA Hood explained the purpose of the inspection was to deliver the complaint findings. LPA observed four infants with two teachers and one aide in the infant classroom and four toddlers with one teacher and one aide in the toddler option classroom. Today, LPA interviewed two children and four staff.

During the investigation, LPA Hood interviewed the director, licensee, the Reporting Party (RP), daycare parents, daycare children and daycare staff. During interviews, it was disclosed the unqualified staff were left alone with the children in the toddler option classroom and infant classroom. LPA conducted a records review which indicated the two staff left in the classroom were not qualified to be left alone with children in care. On 11/07/2024, during a tour of the facility playground, LPA Hood observed that the temporary green fencing was not attached to the wooden pole which could allow access to the off-limit area where hazardous items were observed.

Based on interviews, record review and observation, the preponderance of evidence standard has been met; therefore, the allegations are found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 & Chapter 1, are being cited on the attached LIC 9099D.

An exit interview was conducted, and the report was reviewed with the director Sharifa Osman. The director was provided with a copy of their appeal rights (LIC 9058 3/22) and their signature on this form acknowledges receipt of these rights. Notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

The director stated she will provide the LPA a written procedure plan for the staff or director when conducting a playground sweep. The director will submit the plan to the LPA no later than 02/05/2025.

On 01/07/2025 at 12:50 pm, Licensing Program Analyst (LPA) Michelle Hood conducted an unannounced complaint inspection regarding the above allegation, LPA met with the Director. LPA Hood explained the purpose of the inspection was to deliver the complaint findings. LPA observed four infants with two teachers and one aide in the infant classroom and four toddlers with one teacher and one aide in the toddler option classroom. Today, LPA conducted two children interviews and four staff interviews.
During the investigation,the LPA interviewed the director, licensee, Reporting Party (RP), daycare parents, daycare children, and daycare staff. Based on the interviews, daycare parents expressed concerns about the classroom temperature; however, they also noted that the classrooms are equipped with fans and air conditioning. Due to lack of evidence, the above allegation is found to be UNSUBSTANTIATED meaning that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. An exit interview was conducted, and the report was reviewed with the director Natalia Ellis. The director was provided with a copy of their appeal rights (LIC 9058 3/22) and their signature on this form acknowledges receipt of these rights. Notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. This is an amended version of the original report created on 01/07/2025.
