On 08/14/2024 at 1:35pm, Licensing Program Analyst (LPA), Selina Siao conducted an announced Pre-Licensing inspection. Upon arrival, LPA met with facility representative Diana Hurtado and her son. Applicant is requesting to be licensed for 5 infants (ages 0 to 24 months) in the Infant Care Room. And 24 preschool children ages 3 to kindergarten in classrooms #2A/#2B, #3, #4 and #5. Facility hours of operation is Monday to Friday from 6:30am to 5:30pm.

All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Diana Hurtado that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, facility representative stated that they will be in the individual child's class and it will be placed up high inaccessible to children. A fully equipped first aid kit will be kept in each classroom. There are two operational carbon monoxide detectors on site located at the hallway outside of room #4 and #3. All required licensing documents will be posted in the hall way. Children will be signed in and out via the Brightwheel appl.

LPA continued to tour the facility and measured all indoor and outdoor activity space. Total infant indoor activity space measured to be 277.22 square feet, which is sufficient to accommodate the requested capacity of 5 infants. Total preschool indoor activity space measured to be 2236.80 square feet, which is sufficient to accommodate the requested capacity of 24 preschool children. LPA observed most indoor activity space to be complete with age-appropriate furniture and equipment, including tables, chairs, cubbies, cribs, napping cots/mats for infants who can climb out of a crib, bookshelves, and other activity supplies for the children. Toys are safe with no sharp edges, splinters, or points, nor made of small parts that can present a choking hazard. Drinking water will be available in the classrooms via a water pitcher and facility will be ordering water from the local water supply company. LPA observed all hazardous items to be inaccessible to children. Facility representative stated that there are no bodies of water or weapons on the property. Fire clearance was granted on 07/23/2024 for 5 infants in the infant room and all other rooms are approved for children ages 3 and older only.

Highchairs and feeding tables have broad-based legs, plastic seats are in good repair and trays lock onto chairs. Highchairs are made of washable, moisture-resistant material. Changing tables that was ordered has at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high and the changing table will be placed within arm’s reach of a sink. Diapering sink is not used for meal preparation or dishwashing. Cribs meet regulatory requirements. Crib area is separated from activity space via a door and wall in a separate room. Facility representative was advised that visual supervision is required at all times including nap time. Sleep logs, Needs and Services Plans and Safe Sleep Plans will be kept in the infant care room.

Facility representative stated that the children will be potty train when they transition to the preschool. LPA observed a total of 5 sinks, 5 toilets and 4 urinals available for the preschool children’s use. These are sufficient to accommodate the requested capacity of 24 children. There is a separate staff restroom equipped with a toilet and a sink located upstairs. The isolation area for children who are ill will be in the Director's office and one of the sink and toilet in the men's restroom will be use for ill child.



Facility will provide lunch, AM, and PM snacks for the children. Facility will be bringing food from their other facility until the kitchen at this location has the required appliance. The kitchen area and food storage areas were observed to be free of rodents and/or vermin. Snacks will be store in the kitchen and it will be stored separately from cleaning items.

The facility currently has a fully fenced playground/parking lot area which is separate from other components. Facility has a wrought iron and chain link fencing that is at least four feet high. The total square footage for the preschool outdoor activity space is 4011.79 square feet and the infant outdoor activity space measured to be 549.41 square feet, which is sufficient to accommodate the requested capacity of 24 preschool children and 5 infants. Shade is provided via a sail shade at the infant play area and the preschool area needs shade. Drinking water will be available via the children's own water bottles and zippy cups. Facility representative was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

Facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.”

LPA reviewed with facility representative] the LIC 311A, Records to Be Maintained at The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Facility representative was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. “

The following corrections are needed prior to the issuance of the license to care for 5 infants ages 0 to 24 months in the Infant Care Room and 24 preschool children ages 2 to 5 years old in room #2, #3, #4 and #5.

· Outlet covers are needed in all rooms.
· Room numbers shall be placed outside each classroom.
· Have age-appropriate furniture and equipment in classroom #5.
· Have age-appropriate toys and equipment for the infant and preschool outdoor area.
· Shade is required for the preschool outdoor area.
· Infant crib room shall be set up with cribs.
· All licensing posting shall be posted at the facility for parents to see.
· There shall be signs with the name of the facility outside the building.

Licensee Representative Diana Hurtado understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied.
