On 09/13/2024 at 10:05am, Licensing Program Analyst (LPA) Selina Siao conducted an announced pre licensing follow up inspection at the facility. The initial inspection was conducted by LPA on 08/14/2024.

LPA conducted a tour of the facility and the following items has been corrected:

Outlet covers are in all rooms, Room numbers are placed outside each classroom, room 5 have age-appropriate furniture and equipment, there are age-appropriate toys and equipment for the infant and preschool outdoor area, shade is available at both the infant and preschool areas, infant crib room currently has 6 cribs in the crib room, facility has a bulletin board with the required licensing postings and there is a sign with the name of the facility outside the building.

The following corrections are still needed:
  • Purchase a larger fence gate that is sufficient to separate the infant area according to LPA's measurement on 08/14/2024.

· An updated LIC500 Personnel Report and LIC610 Emergency Disaster Plan, Submit a director's packet for Monica Martinez as Rachelle Wise does not have the required infant units, submit an updated staff manuel to include the required In service training per title 22, 101216 ( e), update the parent handbook page 2 (Enrollment – preschool age for Howard Ave should be for 3 to 5 years old), update the admission policies- What is the facility’s criteria for determining appropriate placement given an individual child’s needs (ex. Parent interviews, preadmission appraisal, etc.) (B6, LIC281A page 29, Admission Policies) and submit an updated control of property that matches the current name of the corporation.
