On 08/30/2024 at 10:00am, Licensing Program Analyst (LPA), Selina Siao conducted an announced Pre-Licensing inspection to add an infant component to the existing preschool facility. This facility is a single license and it will be consolidated with the existing license facility number 376701128. Upon arrival, LPA met with Area Manager Lisa Grygera, Site Supervisor Cynthia Carrillo and Assistant Site Supervisor Cheyanne Tarango. Applicant is requesting to be licensed for 18 infants ages 0 to 24 months in classrooms 4, 5 and 6 and be licensed for 60 preschool children ages 3 to 5 years old in classrooms 1, 2 and 3. Hours of operation is Monday to Friday from 7am to 6pm.

All indoor and outdoor activity space utilized for the children was inspected today. LPA informed facility representatives that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Lisa Grygera stated that they will be in the red emergency backpack that is inaccessible to children in the child's own class. There is a fully equipped first aid kit in each classroom inside the red back pack. There is an operational carbon monoxide detector on site located in all the classrooms. All required licensing documents were observed posted by the front door entrance area. Children will be signed in and out at each classroom.

LPA continued to tour the facility and measured all indoor and outdoor activity space. Total infant indoor activity space measured to be 1489.68 square feet, which is sufficient to accommodate the requested capacity of 18 Children. And the preschool indoor activity space was previously measured to be 1882.38 square feet, which is sufficient to accommodate the capacity of 53 children and not sufficient for the requested capacity. LPA observed all indoor activity space to be complete with age-appropriate furniture and equipment, including tables, chairs, cubbies, cribs, napping cots for preschoolers and infants who can climb out of a crib, bookshelves, and other activity supplies for the children. Toys are safe with no sharp edges, splinters, or points, nor made of small parts that can present a choking hazard. Drinking water is available in the classrooms via a distilled Sparklett water dispenser that are in the classrooms via the children's own water bottles or zippy cups. LPA observed all hazardous items to be inaccessible to children.

Facility representatives stated that there are no bodies of water or weapons on the property. Fire clearance was granted on 07/25/2024.

Facility will be holding young infants that are unable to sit up during feeding. Changing tables have at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high and the changing table is within arm’s reach of a sink. Diapering sink is not used for meal preparation or dishwashing. Cribs meet regulatory requirements. Crib area is separated from activity space via a sound wall. This barrier is less than the required four feet high and it is made of sound absorbing material and allows for supervision of napping children. Sleep logs will be kept in the child's class. Needs and Services Plans and Sleep Plans will be kept in the child's file.

There shall be at least one potty chair or toilet for every five potty training infants. LPA observed a total of 5 sinks and 1 toilet available for the infant's use near room 6. These are sufficient to accommodate the requested capacity of 18 children. There is 5 sinks, 6 toilets and 1 urinal available for the preschool children's use. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be in the Site supervisor or Assistant Site supervisor's office. A child that is ill will use the staff restroom that is inside room #4.



Facility will provide breakfast, lunch, AM and PM snacks to the children. The kitchen area currently includes food warmer, refrigerator and sinks. The central kitchen prepares food and it gets delivered to this site. The kitchen food storage areas were observed to be free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. Hazardous items in kitchen are inaccessible to children.

The facility currently has a fully fenced playground area which is separate from other components. The wrought iron fencing is at least four feet high. The infant outdoor activity space is 891.48 square feet, which is only sufficient to accommodate 11 children at one time. The preschool outdoor area measured to be 3089.75 square feet, which is only sufficient to accommodate 41 preschool children at one time. Both infant and preschool outdoor areas are insufficient for the number of children being requested.

A playground waiver to share the playground at different times in the AM and PM due to limited square footage was submitted and shall be approved prior to licensure. Shade is provided via a tree and canopy at the infant outdoor area and the preschool outdoor area has a few big trees and buildings as shade.

There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is no climbing structure on the preschool playground. There is a little slide that is placed on top of a thick one inch turf at the infant play area. There is adequate cushioning in fall zones of the little slide at the infant area. Drinking water is available via the children's zippy cups and water bottles with filtered water. LPA observed all hazardous items on the playground to be inaccessible to children. Facility representatives was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

Facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.”

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

A license for 53 preschool children ages 3 to 5 years old and 18 infants ages 0 to 24 months will be granted upon a final file review.

The following corrections are needed prior to the issuance of the license:
  • Extend the infant walls napping walls to 4 feet high as the current wall is 3 feet.
  • Obtain an approved outdoor playground waiver by the department.
  • Obtain a safety gate to separate the infant and preschool play area.

Facility representatives understands that all proof of corrections must be provided to the Department via pictures within 30 days, or the application may be denied.

A notice of site visit was given to the Site Supervisor and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
