On 03/10/2025 at 10:00am, Licensing Program Analysts (LPAs), Selina Siao and Samantha Clenista conducted an announced Pre-Licensing inspection for adding a toddler component and converting this facility into a single license facility under the current pending facility number. This facility is currently licensed with a preschool 376701113 and an infant 376701114 facility. Upon arrival, LPAs met with Licensee representative/Director Perla Zazueta. Applicant is requesting to be licensed for 60 children including 12 infants (ages 0 to 2 years old) in rooms 3 and 4, 12 toddlers (ages 18 to 36 months) in room 1 and 36 preschool children (ages 2 to kindergarten) in rooms 2 and 6. Hours of operation will be Monday to Friday 6am to 6pm.

All indoor and outdoor activity space utilized for the children was inspected today. LPAs informed Pearla Zazueta that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Perla Zazueta stated that they will be in the child’s room in the red bag or in the red box in the refrigerator. A fully equipped first aid kit is in the Director’s office and all the classrooms also has a first aid kit. There is an operational carbon monoxide detector on site located in the preschool room #2 and in the infant room #3. All required licensing documents were observed posted in the Director’s office. Children will be signed in and out at each room in a red binder.



LPAs continued to tour the facility and measured all indoor and outdoor activity space. Total infant’s indoor activity space measured to be 465.19 square feet, the toddler room measured to be 658.33 square feet and the preschool rooms measured to be 1382.86 square feet. All indoor activity areas are sufficient for the requested capacity of 12 infants, 12 toddlers and 36 preschool children. LPAs observed all indoor activity space to be complete with age-appropriate furniture and equipment, including tables, chairs, cubbies, cribs, napping cots for preschool children and infants who can climb out of a crib, bookshelves, and other activity supplies for the children. Toys are safe with no sharp edges, splinters, or points, nor made of small parts that can present a choking hazard. Drinking water is available in the classrooms via the portable water cooler and it gets refill by the filtered water located at the Life Center. LPAs observed all hazardous items to be

inaccessible to children. Director stated that there are no bodies of water or weapons on the property. Fire clearance was granted on 2/06/2025.

Facility has low profile chairs with broad-based legs, plastic seats are in good repair and trays lock onto chairs. The chairs are made of washable, moisture-resistant material. Changing tables have at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high and the changing table is within arm’s reach of a sink. Diapering sink is not used for meal preparation or dishwashing. Cribs meet regulatory requirements. Crib area is separated from activity space via a separate classroom #4. Sleep logs will be kept in the infant and toddler classroom. Needs and Services Plans and Sleep Plans are kept in the child’s classroom.

There shall be at least one potty chair for every five potty training infants. LPAs observed a total of 1 sink available for infant’s use, 2 sinks and 1 toilet for the toddler component and 3 sinks and 3 toilets for the preschool component. These are sufficient to accommodate the requested capacity. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the Director’s office and the staff’s restroom.



Facility does not provide any food to the children as the children brings lunch and snacks from home. Facility does not have a kitchen.

The facility currently has a fully fenced playground area which is separate from other components. Fencing is made of chain link and wrought iron and it is at least four feet high. The total outdoor area for the infant is 1195.63 square feet, total toddler outdoor area is 1397.28 square feet,, and the total preschool outdoor area is 7323.82 square feet, the three outdoor areas are sufficient to accommodate the requested capacity. Shade is provided via sail shades, trees, and the buildings. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is a climbing structure on the preschool playground for children ages 3 to 12 years old and it is properly anchored, and it has a separate enclosed area. There is adequate cushioning in fall zones of climber provided by rubber chips. Drinking water is available via the children’s own bottles during outdoor activities. LPAs observed all hazardous items on the playground to be inaccessible to children. Director was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

Director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPAs reviewed with director the LIC 311A, Records to Be Maintained at The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

LPAs discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPAs also informed Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Director was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The following corrections are needed prior to the issuance of the preschool single license:
  • Add a gate at the toddler yard to block off the stairs.
  • Add a sign to designate the toddler toilet and toilet sink.
  • Add a sign for the staff’s toilet and sink.
  • Zip tie the removable fence to the gate.
  • Add a safety latch for the changing table cabinet located in room #1.
  • Clear out the toddler toilet and restroom.
  • Add toys for the toddler yard.

Director understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied.

Exit interview conducted and report was reviewed with the Director.
