On 06/13/2022, Licensing Program Analyst (LPA) Murial Han and LPA Kevin Varilla conducted a 10-day initial complaint inspection visit. LPAs initially met with Director of Social Services, Karina Tapia and LPAs were introduced to Human Resource (HR) Director, Preveen Rattan and Chief Executive Officer (CEO), Mary Linde who assisted with the investigation. LPAs explained the purpose of the visit and reviewed the allegation.

Concerning the allegation of facility mishandled resident and staff confidential information, HR Director and CEO denied the allegation. According to the HR Director, the former HR Coordinator left the facility some time in April, and after his/her's departure, the HR Director went into the HR Coordinator's office that was locked but discovered some of the staff files and resident's records in the file cabinets were unlocked. After the discovery, the HR Director immediately locked and secured all the files. Although the files were secured in a locked office, the facility has improved its system ensuring all the file cabinets were locked as well. Furthermore, only the HR director has access to the office. In addition, the facility has validated that the files were not compromised

During today visit, LPAs toured the HR Coordinator's office and observed the office was locked and all the file cabinets were locked as well. After review, this allegation is unsubstantiated.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is UNSUBSTANTIATED.

This report was reviewed and discussed with HR Director and CEO.
