On June 11, 2026, Licensing Program Analyst (LPA) Leong arrived at the facility to close the complaint investigation. LPA met with Director Kerry Tronstad and explained the purpose of the visit.

There were 40 children and 9 staff members during today's visit. Present staff members on site have fingerprint clearance on file.

All relevant information was gathered and analyzed during the LPA investigation, and all parties involved were contacted and interviewed. Based on the information obtained from the LPA investigation, the preponderance of evidence standard has been met; therefore, the above allegations are found to be substantiated.

LPA Leong informed facility representative Kerry Tronstad that this report dated 6/11/2026 document(s) one Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Leong informed the facility representative to provide a copy of this licensing report dated 6/11/2026 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Appeal Rights were provided to the Director, Kerry Tronstad, during today's visit.

A Notice of Site Visit was given and must remain posted for 30 days.

The director stated she will have an one-on one meeting with the staff member involved and will have the meeting documneted and provided to LPA Leong. Thh director also requested Child Care Advocates from CDSS to assist in go over training with this particulat staf member.

Based on interviews, one staff member was observed in several incidents over a period of time involving violations of children’s personal rights. This violation poses an immediate health, safety, and personal rights risk to children in care.
