25 | On 3/18/2026 at 9:00AM., Licensing Program Analyst (LPA) Luis Gomez met with Substitute Administrative Assistant, Marilyn Dunn. The purpose of visit was explained and was for an unannounced- case management inspection, in response to an unusual incident report, submitted on 2/23/2026. The Principal, Tiffany O’Neal arrived during inspection. Present was the principal and 6 staff supervising 22 children. LPA inspected facility for health and safety hazards.
During inspection, LPA conducted site observation, interviews, and record review.
Per incident report, on 2/5/2026, staff (S1) was observed using an inappropriate/ physical intervention with day-care child (C1) during the program's outside play. Per Principal, once school administration was made aware of the incident, immediate action was taken, conducting interviews and collect written statements from parties involved. Principal reported alleged staff member (S1) is currently on administrative leave, as incident is under investigation.
LPA obtained SFUSD investigation documents, including interviews; supporting evidence; staff training material; and CPS Report. Per Principal, child (C1) continues to attend the program.
