25 | On 5/22/2026 at 8:40AM., Licensing Program Analyst (LPA) Luis Gomez met with Administrative Assistant, Alma Portillo. The purpose of visit was explained and was for an unannounced- case management inspection for unusual incident, submitted to the Department on 4/16/2026. The school principal, Danielle Loughridge was contacted during the inspection. Present was Lead Teacher, and 7 staff supervising 34 children. LPA inspected facility for health and safety hazards.
During today’s inspection, LPA conducted interviews with staff and site principal and reviewed facility records.
Per report, On 4/9/2026, child’s guardian arrived at preschool site and was unable to locate child. Per report, child (C1) was found alone minutes later inside classroom #4 (Bathroom). Per Principal, after the incident, immediate action was taken by administration including meetings with the preschool staff and developing procedural changes to ensure classroom doors remain closed during scheduled outside play.
LPA obtained SFUSD investigation documents, including interviews, staff statements, and completed incident reports forms (LIC624).

An exit interview discussing the facility evaluation report and plan of correction was conducted with Lead Teacher, Christy Zheng and signature on this form acknowledges the receipt of these documents.
This report must be made available in facility for public review. Notice of site visit and appeal rights were given and must remain posted for 30 days.
