On 11/25/2024, at 2:00PM, Licensing Program Analyst (LPA) Jonathan Tse and Licensing Program Manager (LPM) Ali Zebila met with Executive Director Dawn Mokuau and Site Supervisor Naomi Miura for a scheduled informal office meeting. This meeting was conducted virtually on Microsoft Teams. A complaint was filed against the facility and investigated by the Department's Investigation Bureau. This informal meeting is a direct result of a Type A citation cited during a complaint investigation on 7/2/2024, under HSC 1596.99(c)(1).

During today's meeting, LPM Zebila discussed and reviewed the complaint findings:
  • A violation of HSC 1596.99(c)(1)

The use of age-appropriate toys and materials was discussed. Reporting requirements were discussed with the facility representatives during the visit.

Since deficiencies were cited, the facility has:
  • Conducted a staff meeting that covered reporting requirements, supervision requirements, and age requirements of play equipment

Facility representatives understand that the Department will increase monitoring of the facility for the next 12 months. Executive Director agreed and stated that the facility plans to stay in compliance with regulations. Assembly Bill 633 (Childcare Parent Notification Requirements) and Acknowledgement of Receipt of Licensing Reports (LIC9224) was discussed.

This report was emailed to the facility email. Exit interview conducted and report was reviewed with Executive Director Dawn Mokuau and Site Supervisor Naomi Miura.
