LPA reviewed four staff files, five children’s files, and facility records. All staff present have current Pediatric First Aid/CPR training. All staff have completed Mandated Reporter Training. Staff qualifications and immunization records are available for review. LPA advised that all staff members shall have a Health Screening Report (LIC503) completed and signed by a physician. Children’s files were reviewed and observed to contain Notification of Parents’ Rights (LIC995) and Emergency Identification and Information (LIC700). LPA advised that Child’s Preadmission Health History (LIC702) shall be maintained in each child’s file or included in the facility’s enrollment application. Licensee stated that they understood. Admission agreements are stored digitally and available for review.
LPA observed required postings to be posted and accessible for review. LPA advised that the Earthquake Preparedness Checklist (LIC9148) shall be posted. Licensee stated that they understood. Emergency drill logs are to be stored and available for review for at least one year. Per Licensee, drills are conducted once a month. The facility tested its fixtures for lead and remediated one fixture that had an exceedance of lead. The facility is in compliance with lead testing requirements as of today, 9/19/2024. LPA advised that lead testing shall be conducted every five years.

Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a
maximum of 30 days per person will be assessed if this regulation is violated.
