On January 24, 2025, at approximately 9:15am, Licensing Program Analyst (LPA) Ly conducted a Case Management Inspection at the facility due to a self reported incident that happened on 01/09/2025. Incident was reported by phone to Licensing on 01/10/2025 and followed with a written report from facility representative. During today's visit, LPA met with facility Center Manager Rachele Long. There were 12 children and 6 staff including the Center Manager present during today's visit.

During today's visit, LPA interviewed Center Manager, Regional Manager and Witnesses regarding the self reported Unusual Incident. The involved staff has been placed on Administrative Leave.

Based on interviews with witnesses and information collected, the department has determined facility staff violated the children's personal rights. Type B deficiency is being cited today in accordance with the California Code of Regulations, Title 22, see LIC 809D.

Plans of Corrections (POC) were developed and reviewed with Center Manager. A copy of this report was discussed and left with Center Manager whose signature on this form confirm receipt of reports. Notice of Site Visit was provided to licensee. Center Manager was advised Notice of Site Visit to remain posted for 30 days. For updates on Licensing information, go to CCL website: www.ccld.ca.gov. Regional Manager stated facility will send involve staff to trainings. The Center Manager will review Personal Rights requirement with staff during staff meeting.

Based on interviews collected during today's visit, the involved staff had violated the children's personal rights which poses a potentital health and safety risk to children in care.
