On 11/14/2024, at approximately 1:40PM, Licensing Program Analyst (LPA) Jonathan Tse conducted an unannounced case management visit at the facility. LPA met with co-teacher Winnie Richardson and explained the purpose of the visit. Present during the visit were two teachers and nine preschool age children.

The Department received an incident report from the facility on 11/13/2024 detailing an incident where a child expressed that they did not want to rest, and was placed on a napping bed instead of being allowed to continue a quiet activity by a staff member (S1). This is a self-reported incident.

LPA conducted staff interviews and site observations during the visit. LPA arrived during nap time. Children were observed to be napping or having a quiet activity. LPA found that the self-reported incident described a violation of a child's personal rights. Based on interviews with staff, LPA found that corrective action was taken with staff members involved and that staff are now aware of children's personal rights. LPA acquired a copy of the discussion between the Principal and S1. Based on interviews with staff, LPA found that staff members offer quiet activities for children who are not napping during designated nap times.

See LIC809-D for deficiency cited and cleared today regarding children's personal rights.

A notice of site visit was provided and must remain posted for 30 days. Appeal rights were provided and explained to Principal Frazier. Exit interview conducted and report was reviewed with Principal Don Frazier.

Based on interview and record review, the licensee did not comply with the above by placing a child on their napping bed even though the child expressed that they did not want to nap. This posed a potential risk to the health, safety, or personal rights of children in care.
