On 7/12/2024 Licensing Program Analysts (LPAs) Basak and Leong met with the Director, Sandra Sandy McKeithan for an unannounced annual inspection in conjunction with a case management visit. The purpose of the inspection was explained, and LPAs were granted entry. 7 staff and 11 preschool children were present during the visit. LPAs observed the facility was operating within capacity and complied with the staff/child ratio. The facility operates from 8:00 am to 5:00 pm Monday- Friday.

According to the director, the facility uses an electronic sign-in/out app, Care Connect. LPAs observed all authorized representatives have signed. The facility has a license, and all other required documents are posted and visible to the public. The facility has a functioning telephone.



LPAs inspected 2 preschool classrooms with the director. LPAs observed facility has combo smoke and carbon monoxide detectors, and fully charged 2A10BC fire extinguishers in multiple areas, including the classrooms and hallway. LPAs verified with the director the facility has an emergency drill log, and the last drill was conducted on June 28, 2024. Per the Director, drills are conducted once a month.

LPAs observed all cleaning supplies and other chemicals are stored in a locked cabinet and inaccessible to the children. LPAs verified the facility has a fully stocked first aid kit available in the classrooms.



According to the director, there are 2 children with allergies at the time. LPAs checked and observed that the medications were in their original box, updated, included consent forms, and properly stored in a locked container.

LPAs observed the facility has age-appropriate furniture, play, and educational materials for children in each classroom. The facility floor and rugs are in good condition and free of any hazards.

LPAs observed all bathrooms are sanitary and in working condition. There were 3 toilets and three sinks shared between classrooms and the changing table by the sink. The water temperature was proper. The facility has enough sleep cots/mats for children, and each has individual sheets and blankets. According to the director, the facility provides breakfast, lunch, and afternoon snacks, and drinking water from filtered water jars.

LPAs inspected and measured the new outdoor play area at the De Marillac Academy. The inspection included measuring the outdoor play area at the De Marillac Academy, located right next to the facility. The combined usable area in the outdoor space is 2,080 square feet, which provides space for 28 children in accordance with the licensing requirement of 75 square feet of outdoor activity space per child.

The outdoor play area was surrounded by a fence and was observed to be at least four feet high. According to the director, it will be used for free play. LPAs observed safety gates and benches available to block the entrances of the staircases in the outdoor play area. LPAs also observed the play equipment, such as hula hoops, balls, stepping stones, and scooter boards, available for them to use and found it maintained in a safe condition, free of sharp, loose, or pointed parts.

LPAs reminded the director that areas around and under high climbing equipment, swings, slides, and other similar equipment must be cushioned with material such as sand, wood chips, pea gravel, or rubber mats that absorb falls.

The San Francisco Fire Department inspected the outdoor play area at the De Marillac Academy and approved the facility's use of it.

LPAs will approve the facility to use the outdoor play area at the De Marillac Academy.

The LPA reviewed the facility’s records, 4 random children files, and 4 staff files. All required papers/forms were available in the children’s folder, including Immunization records, LIC 700. LPAs reviewed the qualification requirements of the staff, CPR, and Mandated Reporter Training and, LPA observed the completion certificates on file.

The director is aware that all staff is required to complete Mandated Reporter Training every two years. LPA advised the director the training can be obtained online at www.mandatedreporterca.com

LPA encouraged the director to frequently visit our website at www.ccld.ca.gov for licensing regulations and new updates.

The director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

The facility was informed that as of September 1, 2016, a person may not be employed or volunteer at a childcare facility unless he or she has been immunized against influenza, pertussis, and measles or qualifies for an exemption pursuant to Health and Safety code 1596.7995 and 1597.662.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5 years after the date of the first test.

For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

The Director was informed of the MyChildCarePlan.org website, a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

No deficiencies were cited today under CCR, Title 22, Division 12, Chapter 3.
