On June 21, 2024, Licensing Program Analyst (LPA) Ly conducted an Unannounced Case Management-Licensee Initiated in adding co-licensee Vinson Chen whom is Licensee's son to her current license. Upon arriving at facility LPA was greeted by 2 Assistants. Assistants stated Licensee and co-licensee just step out. Licensee returned to facility 5 minutes after receiving call from Assistant. Co-Licensee also returned to facility 5 minutes into inspection after receiving call from Assistant. There are 4 infants and 5 preschoolers present during today's visit.

During today's visit, LPA toured the facility and observed co-licensee has his own bed next to Licensee's bedroom.

During today's visit, co-licensee has been advised of the following before officially be added to the license as co-licensee:
*Update facility sketch.
*Fully moved in.

No deficiencies were issued today under Title 22 Division 12 of the California Code of Regulations A copy of this report was discussed with Licensee whose signature on this form confirm understood the reports. Due to printer malfunction, LPA will email report to Licensee by the end of today. Notice of Site Visit will also be emailed to licensee.
