On 6/20/2023 at 9:00A.M., Licensing Program Analyst (LPA) Luis J. Gomez met with Director, Antonette Green, Director of Facility, John Castro, and Head of School, Shelly Willie. Purpose of inspection was explained and was for Announced Case Management inspection for Room Change / Decrease in Capacity. Per Director facility will be re-opening on 6/23/2023.

Facility has requested a capacity of 56 children (Preschool-age) and to add three classrooms to the preschool program. Classrooms are located in St. Joseph’s Hall, #109, #113; and the Annex Building’s, 16th Street Classroom. Facility has request to add two, shared, outdoor play spaces, located at the Boys and Girls Club (450 Guerrero Street, San Francisco, CA, 94110) and lower schools, on-site playground. Day and hours of operation Monday- Friday from 8:00 am to 6:00 pm. LPA measured and inspected entire facility, indoors and outdoor, for health and safety hazards.

At 9:10AM., LPA measured classrooms #109, #113 and 16th Street Classroom in the Annex Building. Total useable indoor square footage of all three classrooms is 1990.55 sq ft; divided by 35 sq ft equals to 56 children. Per director, program is planning to have total of five toilets and four sinks available for the children.

Exclusive use, preschool bathrooms, are in located in St. Joseph’s Hall and Annex Building. Sink deliver water that is no warmer than 120 degrees. There is a separate bathroom for staff usage on the premises. Total allowable capacity for toilets is 75 and sinks is 60.

Each classroom was observed clean, with furniture, playthings, and equipment in safe condition. LPA observed storage cubbies for children's belongings. For napping services, LPA observed cleanable mats. Site has a fire alarms and sprinkler system throughout the facility. Classrooms are equipped with fire extinguishers (2A:10BC) and smoke detectors (Built-in). Accessible trash bins outlets have been coverings installed. First aid kit and emergency supplies are in each classroom. The facility has proper ventilation and lighting. (REFER TO 809C FOR CONT.)

Outdoor: Total useable outdoor square footage of the outdoor play area, located at the on site, Lower Playground, and at the Boys and Girls Club, measures 10,332.88 sq ft; Total area divided by 75 sq ft allowing for a total of 137 children.

Outdoor play spaces were enclosed by a fence over four feet tall. Play spaces were observed clean, safe, and equipped with age-appropriate toys and equipment for children. Both areas have resilient material (turf and tan bark) on floor/ ground surface for added safety. Play structure in lower school was age-appropriate. Shading rest areas are accessible both outdoor play spaces. Per director, water services will be readily available with pitches and refillable bottles.

Children will bring their own lunch and snacks daily. LPA reminded director to ensure food items are properly labelled. Medications will be stored in each classroom inaccessible to children. Drinking water is available for children indoors and outdoors. All toxins and other hazardous items will be stored in each classroom inaccessible to children. Isolation of ill children will be in the lobby.

Facility provides Incidental Medical Services – IMS. LPA reviewed storage area for medication and equipment/supplies, and reviewed forms that will be used. Fire Clearance Request was granted on 5/9/2023.

During inspection, waiver request letter were submitted for rotational use of play yard, and regular use of off-site play space located at the Boys and Girls Club. .
