On 6/23/2023 at 1:45PM., Licensing Program Analyst (LPA), Luis J. Gomez met with Director, Antonette Green and Facilities Manager, John Castro. Purpose of the inspection was explained and was an announced follow-up, case management inspection, to review corrections established on 6/20/2023. Facility is applying for decrease in capacity/room change. Present was the director and no children. Program is planning to utilize the following classrooms: St. Joseph’s Hall: Classroom #109, #112; and 16th Street Classroom in the Annex Building. LPA inspected facility with director, for health and safety hazards.

At 1:50PM., the following was observed: Children’s bathroom, located in St. Josephs Hall, was maintained clean with adequate supplies for hand washing. Fixtures tested were in operating condition.

LPA inspected rooms #112, 16th Street classroom. Functioning carbon monoxide detector had been installed in each classroom.

At 2:00PM., LPA confirmed hallway doors in Annex Building, 16th Street classroom have been made inaccessible with safety locks.

Per director, facility will retracting the pending waiver request, adding the off-site, Boys and Girls Club to the outside play space. Per director, program is planning to share the lower school playground with the elementary school. Updated schedule has been submitted to the department. Lower school playground measures a total usable space of 4570 sq. ft. which divided by 75, can accommodated a total of 60 children.

LPA observed required posting in classrooms, including the Notification of Parents’ Rights (PUB269), Child Passenger Restraint System (PUB269), Emergency Disaster Plan (LIC610), and Personal Rights (LIC613A).

On 6/22/2023, Director submitted proof of purchased napping supplies to accommodate licensed capacity.

On 6/23/2023, Facility will be recommended for approval for Decrease in Capacity/ Room Change.

>No deficiencies were cited against the facility under CCR, Title 22. 12 Ch. 1.

Copy of the report was reviewed with director. This report will be kept with the facility files and made available upon request.
