25 | On October 9, 2025, Licensing Program Analysts (LPAs) Leong and Van conducted an unannounced case management visit to follow up on an incident involving a child. LPAs met with Director Dale Kleisley and Assistant Director Jamie Falkenhainer and explained the purpose of the visit. On October 7, 2025, the department attempted to visit the facility, but the facility was closed for a religious holiday.
Fourteen staff members supervised forty-nine children during today’s visit.
Based on record review, the incident occurred on September 17, 2025, and the facility reported it on September 18, 2025, meeting the department’s reporting requirement by notifying the department within 24 hours.
Based on record review, the department received the Unusual Incident Report on September 23, 2025, meeting the department's requirement to submit a written report within seven days.
According to the facility incident report, the child tripped over a small rubber ball, and the staff initially found no swelling. Later in the day, staff noticed that the child’s left foot was swelling, and the facility contacted the father immediately. The child’s mother came to pick him up to bring him to see a doctor.
According to the director, the child’s doctor authorized the child to return to the facility after the incident with staff observation. However, the parent decided to keep the child at home and return the child to the facility on September 29, 2025.
LPAs found that the facility followed the department’s regulations involving this incident with the child.
The incident will be closed during today’s visit.
A notice of site visit was given and must remain posted for 30 days.
