25 | On November 4, 2024, Licensing Program Analyst (LPA) Van met the Site Coordinator, Abdulmobdy Monim, for an unannounced Plan of Correction (POC) inspection. The purpose of the inspection was explained, and entrance to the center was granted. Five teachers supervised 62 school-aged children today.
During the annual inspection conducted on October 17, 2024, the Center was found to have several deficiencies. These included the Center's failure to conduct water lead testing before January 1, 2023, as mandated by the Written Directive. Additionally, not all teachers had completed the required mandated reporter training, and there were missing immunization records. Furthermore, one teacher had no criminal record clearance before starting work at the Center.
During today's inspection, LPA confirmed that all staff have completed the mandated reporter training and that immunization records are current. Furthermore, water lead testing has been conducted, and the previously uncleared fingerprint teacher has now been cleared and associated with the Center. As of today, all deficiencies cited during the inspection on October 17, 2024, have been resolved.
No deficiencies were observed in today's inspection.
A notice of site visit was given and must remain posted for 30 days.
An exit interview was conducted, and the report was reviewed with the Site Coordinator, Abdulmobdy Monim. |
