25 | On January 21, 2026, Licensing Program Analysts (LPAs) Leong and Ly arrived at the facility for an unannounced case management visit. The purpose of the visit was to follow up on a self-reported incident reported by the facility. LPAs met with Director Corina Hill (S1) and explained the reason for the visit.
During today’s visit, 4 staff members supervised 59 children.
On January 7, 2026, three children left the school premises while in the care of the YMCA program. According to the incident report, the children were brought to the principal’s office, and a school district staff member escorted the children back to S1. The incident report also indicated that none of the YMCA staff observed the children leaving the premises.
The department determined that there was an absence of supervision when the three children left the site and walked outside the school grounds without staff knowledge. This determination was based on staff interviews and the incident report received on January 15, 2026.
A $500 civil penalty was assessed today.
LPA Leong informed facility representative Corina Hill that this report dated 1/21/2026 document(s) one Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.
Also, LPA Leong informed the facility representative to provide a copy of this licensing report dated 1/21/2026 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
