25 | On February 5, 2026, Licensing Program Analyst (LPA) Leong arrived at the facility to conduct an unannounced Plan of Correction (POC) visit. This visit was a follow-up with a previously cited deficiency. LPA met with the director, Corina Hill, and explained the purpose of the visit.
At the time of visit, the facility was staffed by the director and 5 staff members, with 49 children in care
On January 21, 2026, the facility was cited for the absence of supervision involving three children.
LPA received additional POC information from the Executive Director regarding this incident. The following information was received on February 2, 2026.
During this meeting, we formally reviewed our Plan of Correction (POC) and reinforced applicable policies and procedures with staff regarding transitions, supervision, and sign-in/out. As part of our corrective actions, staff have been assigned to designated stations at the front door to ensure proper supervision and staff sign-in/sign-out procedures. Staff have also been assigned to pick up TK students directly from their classrooms and transport them to the Multipurpose Room/ASP Program.
During today’s visit, an Office Meeting letter was provided to the director. The letter informs the facility that a YMCA representative must attend an Office Meeting at the San Bruno Regional Office on February 20, 2026, at 2:00 p.m. This meeting is regarding the deficiency cited on January 21, 2026.
The director asked whether she needed to bring any additional POC documents to the meeting, and LPA advised that it is recommended.
LPA will also request that the director bring the LIC 9224 form for all enrolled children to the meeting.
LPA observed that the Notice of Site Visit and the Type A deficiency page from January 21, 2026, were posted, meeting the department’s posting requirements.
A notice of site visit was given and must remain posted for 30 days.
