25 | On 9/24/2024 at 12:45PM. Licensing Program Analyst, (LPA) Luis Gomez met with Principal, Sarah Twiest. The purpose of today's inspection was explained and was for unannounced, case management inspection, in response to an unusual incident report. Present was the Principal and 6 staff supervising 36 children. Children present had been properly signed in. Facility was inspected for health and safety hazards.
On 8/29/2024, facility submitted an unusual incident reported to the Department. Per incident report, child in program, C1, reported being inappropriately touched by staff while in care. Per principal, an internal investigation was conducted with SFUSD administration. As part the school district's investigation, interviews were completed with Staff; Guardian; and Children.
During today's inspection, LPA conducted record review, observation, and staff interviews.
During interviews, staff reported only using appropriate intervention methods when assisting children with behavioral needs.
LPA advised principal to submit an updated Unusual Incident Report (LIC624) to the licensing office. Copy of investigation report and pertaining documents was provided by principal during inspection.

Facility Evaluation Report was signed by Teacher, Lilian Ruano.
This report must be made available in facility for public review. Notice of site visit was given and must remain posted for 30 days.
