On November 8, 2024, Licensing Program Analyst (LPA) Van conducted an unannounced annual inspection and met with site Director Mimi Sarkisian. The purpose of the inspection was explained, and access to the Center was granted. This facility is an after-school program for school-aged children, operating at R.L. Stevenson Elementary School, which is a functioning SFUSD school site. The program is conducted in the school cafeteria. The operating days and hours are as follows: Monday, Tuesday, Thursday, and Friday from 2:05 PM to 6:00 PM and Wednesday from 12:50 PM to 6:00 PM.

LPA inspected the facility for potential health and safety hazards. LPA observed the children lining up and being checked in by the Director. During check-in, a staff member also handed out supper to the children. According to the Director, the school district provides supper to all after school programs. The cafeteria has good ventilation and ample lighting. The center has a centralized smoke detector, fully charged fire extinguishers, and a carbon monoxide detector.

LPA observed age-appropriate toys and art supplies present. All poisons and chemicals are stored inaccessible to children. The facility shares bathrooms with the school. The girls’ bathroom has four toilets and three sinks. The boys' bathroom has five urinals, two toilets, and three sinks. All toilets and hand-washing stations are in good working condition and good repair.

The outdoor playground area is in good repair and completely fenced for supervision. The play structure is age-appropriate in good condition, and free of debris.

Based on the record review, the Center did not comply with the abovementioned section. All staff members have not completed their Mandated reporter training, or the training has expired, which poses/posed a potential health, safety, or personal rights risk to persons in care.

During today’s inspection, LPA reviewed children's and staff records. All children had their required licensing forms completed, and every staff member had undergone a criminal record clearance. However, it was noted that the mandated reporter training for all staff members had expired.

All required postings (i.e., License, waivers, Notification of Parent's Rights, and Notification of Personal Rights) were observed. The Director stated that Fire and Earthquake drills are conducted at least once every six months, with the latest drills completed on 9/7/2024.

The incidental Medical Services (IMS) policy was discussed. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Childcare Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Childcare Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA reminded the Director that as of September 1, 2016. A person may not be employed or volunteer at a childcare facility unless he or she has been immunized against influenza, pertussis, and measles.
The Director was informed about the Mandated Reporter training that is available on the CCLD website. In addition to AB1207 training, which must be completed every two years by all staff hired. LPA encouraged the Director to visit the CCLD website at www.ccld.ca.gov for licensing regulations and new updates. The Director can also email childcareadvocatesprogram@dss.ca.gov and ask for the updates to be added to the email list.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

The Center currently has a pending request for a waiver regarding water lead testing with the San Bruno Regional office.
