On 8/23/2024, at approximately 1:40PM, Licensing Program Analyst (LPA) Jonathan Tse conducted an unannounced annual visit at the facility. LPA met with Director Vasi Thathiah (D1) and explained the purpose of the visit. Present during the visit was D1, five staff members, and 13 infants. The facility is in compliance with staffing and ratio requirements on this day. The facility’s operating hours are from 7:00AM to 6:00PM.

This facility is a combination infant and preschool center. The preschool center’s facility number is 380505730. LPA and D1 inspected the facility for any health or safety hazards. The facility is equipped with a fully charged 2A10BC fire extinguisher. There are operational carbon monoxide detectors present. There is a fire alarm system running throughout the facility. Emergency exits are clearly marked and visible. Evacuation routes are posted and accessible for review in each room. Poisons, cleaning detergents, and other chemicals are stored inaccessible to children in care.

LPA observed the facility to be in clean and orderly condition. LPA observed age-appropriate toys and learning materials to be present. Furniture was observed to be age-appropriate and free of rough or sharp edges. There are cubbies available for each infant in care. LPA observed there to be two diaper changing stations present. Both diaper changing stations are within hands reach of a handwashing sink. LPA observed extra diapers, wipes, and cleaning materials to be stored accessible for staff to use. Per D1, food and milk are provided from home. LPA observed bottles of milk to be appropriately labeled and stored properly in a refrigerator.

LPA and D1 inspected the facility’s napping room. LPA observed there to be temperature and ventilation controls for the safety and comfort of children in care. LPA observed there to be 10 cribs available. There are mats available for infants who are capable of climbing out of a crib unassisted. LPA observed the following:

1) An infant was swaddled while napping
2) Mesh baby bumpers attached to the side of each crib
3) Blankets and toys located in cribs with napping infants
4) Sleep sacks present in cribs

LPA informed D1 that their observations showed violations of infant safe sleep regulations. LPA discussed infant safe sleep regulations with D1. D1 stated that they understood. D1 and staff removed baby bumpers, blankets, sleep sacks, and toys from each crib. D1 removed the swaddle from the sleeping infant. LPA informed D1 that a deficiency would be cited for swaddling an infant, and that technical violations would be issued for the other violations. D1 stated that they understood. LPA advised that all staff shall be made aware of safe sleep regulations and that parents shall be informed of safe sleep requirements for the facility.

LPA observed the outdoor play area to be free of debris and other loose articles. LPA observed there to be age-appropriate toys and equipment available for children in care. LPA advised that D1 shall ensure that children from either center are not co-mingling during outdoor play. D1 stated that they understood. There are no pools or other similar bodies of water present in the facility.

LPA reviewed five staff files, eight children’s files, and facility records. All staff have current Pediatric First Aid/CPR and Mandated Reporter training available for review. Staff qualifications and transcripts are available for review. LPA advised that all staff shall have immunization records available for review. D1 stated that they understood. LPA observed all children’s files to be complete, including Individual Infant Sleeping Plans (LIC9227). Infant sleeping logs are available for review. LPA advised that sleeping logs shall be kept for all infants in care. D1 stated that they understood.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a
maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
