On May 09, 2023 at approximately 12:35pm, Licensing Program Analyst (LPA) Ly arrived at facility and met with Regional Manager Daisy Hsieh and Site Director Fanny Lee to close this complaint. The purpose of the inspection was explained. Present during the visit were Regional Manager, Site Director and 10 staff caring for 20 children.

During the course of investigation, LPA reviewed records, conducted interviews, and obtained copies of pertinent documents. Based on information gathered,there was no sufficient evidence to prove allegation Day care child sustained an unexplained injury while in care. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is UNSUBTANTIATED.

A copy of this report and notice of site visit were discussed and left with Site Director Fanny Lee whose signature on this form confirm receipt of these reports. Notice to remain posted for 30 days.
