25 | On 7/30/2025 at 12:45PM., Licensing Program Analyst (LPA) Luis Gomez and met with Director, Patricia Bacerra. The purpose of inspection was explained and was for an unannounced, plan of correction inspection. Present was the director and 2 staff supervising 14 children. LPA inspected facility, indoors and outdoors, for health and safety hazards.
During inspection, LPA conducted record review, observation, and interview.
At 1:00PM., LPA review sandbox in the outdoor play area. The sandbox was free of hazardous material, with playthings in good repair.
Director stated adhesive on padded covering was ineffective and would not stay on textured surface. Going forward, director will use an indicator (florescent tape), to indicating where children can/ cannot play while in the sandbox. Photo taken by LPA during inspection.
LPA reminded director to ensure staff are made aware of new sandbox policy, and practices of supervising/ enforcing sandbox policy, are incorporated into the daily routine.
During visit, deficiencies issues during annual inspection was cleared, and 'letter of deficiency citation cleared' was provided to licensee.
An exit interview, and facility evaluation report was discussed with Director, Patricia Bacerra. Director’s signature of this form acknowledges the receipt of these documents.
