Three full-size pack 'n plays were observed in the nap room and found to have thick folded blankets used as mattresses. The licensee explained that the boards that came with the pack 'n plays were hard and uncomfortable, which is why she used thick blankets for padding. LPAs reviewed the infant safe sleep regulation and reminded the licensee that all bed sheets must be tightly fitted to prevent dislodging, and blankets should not be used because they could come loose and pose a strangulation or suffocation hazard during an infant's nap. A technical violation was issued.
The infant sleeping logs were provided to LPAs for review during the inspection. Upon review, it was found that the licensee maintained complete logs, including the child's name, date, and the 15-minute increment checks. LPAs reminded the licensee to maintain a minimum of 30 days of logs for review during the annual inspection, and the records must be kept for 3 years.
Cleaning products, such as disinfectant and Clorox, were observed stored under the sink without a child-proof lock. The licensee immediately removed these items and placed them on high shelves. LPAs reminded the licensee to ensure all cleaning products are locked or stored out of children's reach. A technical violation was issued.
The outdoor deck is completely fenced and well-stocked with a variety of outdoor toys and equipment in good condition. No hazards were observed.
Per licensee, there are no firearms, weapons, or pets in the home. LPAs observed no pools, spas, or other bodies of water on the premises. All poisons, detergents, and cleaning products are stored out of reach for children.
LPAs observed that the licensee had posted all the required forms (i.e., License, Emergency Disaster Plan, etc.). Fire drills are conducted and documented accordingly, and the last one was held on 10/10/2025.
Random children’s records were reviewed during the visit. Licensee has records for children in care, including immunization records and Parents' Rights forms, and emergency contact information.
The licensee and her assistant have up-to-date Pediatric First Aid, mandated reporter training, and immunization records.
