25 | On June 9, 2026, Licensing Program Analyst (LPA) Van arrived at the family child care home to conduct an unannounced Plan of Correction inspection. LPA met with the licensee’s assistant, Nino Osepashvili, and explained the purpose of the visit. The co-licensee, Ruzana Mamysheva, was not present. Upon entry, the licensee, Liliana Goldenshteyn, and two assistants were supervising eight children (three infants and five preschool-aged children).
During the annual inspection conducted on May 19, 2026, LPA observed violations related to infant safe sleep and staff immunization requirements. LPA observed one of the three cribs with an oversized and loose bed sheet, and blankets placed inside multiple cribs and playpens. LPA also observed that one licensee’s immunization record was incomplete, with no documentation of pertussis (Tdap) immunization on file.
During today’s visit, LPA verified through observation and record review that all previously cited deficiencies have been corrected. The licensee ensured that all cribs and playpens were free of loose bedding and soft items, and LPA confirmed that safe sleep equipment was set up in compliance with requirements. The licensee also updated staff immunization records, and LPA verified that complete documentation, including Tdap, is now maintained in the record.
Based on today’s inspection, all deficiencies cited on May 19, 2026, have been corrected, and the family child care home is now in compliance.
A Notice of Site Visit was issued and must remain posted for 30 consecutive days.
An exit interview was conducted, and the report was reviewed with the licensee, Liliana Goldenshteyn. A copy of the report was provided.
