25 | On May 9, 2025, Licensing Program Analyst (LPA) Leong conducted an unannounced Plan of Correction (POC) visit to follow up on deficiencies cited on March 26, 2025. LPA met with Licensee Marlis Harris and explained the purpose of the visit.
Nine pre-k children, the licensee, and the licensee’s assistant were present at today's visit.
On March 26, 2025, the facility received two deficiencies. The first deficiency was for not filling out form LIC 9224, notifying parents or guardians of the enrolled children about a Type A deficiency within 24 hours of the deficiency. The second deficiency was the licensee's absence from the facility for 80 percent of operating hours. The licensee was on vacation when LPA visited her facility on March 26, 2025
LPA reviewed form LIC 9224 and found that all twelve enrolled children’s parents or guardians had signed and acknowledged that the facility had received a Type A deficiency on March 19, 2025.
LPA discussed with the licensee about the regulation that the licensee must be present in the facility for 80 percent of operating hours. The licensee understood, and moving forward, she will notify the department when she takes time off from the facility.
LPA reviewed the licensee’s Pediatric CPR and First Aid certificate and found it was completed on April 30, 2025, from the American Red Cross. The certificate will expire in April of 2027.
LPA also reviewed the Mandated Reporter Training certificate and found it was completed on November 20, 2024. The certificate will expire on November 20, 2026.
The two deficiencies cited on March 26, 2025, will be cleared during today’s visit.
A Notice of Site Visit was given and must remain posted for 30 days.
