25 | On April 19, 2024, Licensing Program Analyst (LPA) Van conducted case management to follow up on a pending Attestation Request for water lead testing and met with the lead teacher, Gelica DeGuzman. The purpose of the inspection was explained, and the staff granted LPA entrance to the Center. Four staff members were present supervising 14 preschool children. The Director, and several more children arrived during the inspection. The Center complied with a teacher/child ratio.
Based on the previous case management on March 8, 2024, the center sketch and physical plan indicate a water fountain close to the children's restroom, accessible to children. However, the Director stated that the water fountain had been turned off and unused. The Center used duct tape to cover the side handlebar and bubbler to prevent children from accessing it.
During the inspection today, it was observed that the Center had installed a 5-foot tall cabinet standing upright and placed in front of the water fountain. The cabinet was observed to be sturdy and was secured with a combination lock. Furthermore, the back of the cabinet was securely mounted against the wall to prevent it from tipping over. LPA reminded the director to keep the cabinet locked during the center's operating hours and ensure the water fountain is inaccessible to children.
The Attestation that the Center only uses water delivered and does not use the fixtures in the facility to provide drinking water or prepare food will be pending review by management. The Attestation letter of approval will be sent to the Center upon approval.
A notice of site visit was given to the director, and it must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
An exit interview was conducted, and the report was reviewed with the Director, Galina Cherkas. |
