25 | On May 11, 2023 at 2:00pm, Licensing Program Manager (LPM), Garfield Leung and Licensing Program Analyst (LPA), Winnie Ly met with Bright Horizons Regional Manager Catherine Trabanino and Interim Center Director Aamena Alloo for an Informal Office Meeting.
Discussed during the meeting was Unusual Incident happened 04/19/2023 in which 3 toddlers were able to open the emergency exit gate in the yard and walked out of the facility. Children were found by bystander on the sidewalk of the center. Per Interim Director, children were out by the sidewalk for an approximate 3 minutes. Per Interim Director, family of children were notified and informed of the incident.
Facility took immediate action and Plans of Correction presented and submitted to Licensing. The Plan of Corrections are as follow:
1. Moving door handle up higher; which is currently 52 inches from the ground making it inaccessible to children. UCSF Fire Marshall have checked and approved the door handle to be moved higher.
2. Everyday before children go out to the yard, staff will make sure alarm and surveillance system are working. The system is being checked 3 times a day.
3. Staff have been retrained and reminded to make sure to always maintain sight and sound supervision of children in care.
4. During today's meeting, facility provided all correction documents to Licensing.
Also discussed during this meeting, LPM Leung acknowledged the important action facility taken in reporting unusual incident to Licensing and to parents. LPM Leung also discussed the citation and civil penalty issued for this incident. Furthermore, facility representatives were advised facility will be closely monitor by Licensing that facility adhere to the safety protocols discussed during this meeting to ensure the safety of the children in care.
Citation issued during Case Management Visit on 04/26/2023 has been cleared with today's Informal Office Meeting.
