25 | On January 10, 2025 at approximately 11:00am, Licensing Program Analysts (LPAs) Ly and Naves conducted a Plan of Correction (POC) Visit and met with Director Alice Ho Seher. Purpose of visit was explained. Present during the visit were 11 staff including the Director caring for 46 children.
The POC is regarding Type B deficiencies cited on 12/13/2024:
-Staff did not have proof of completed Child Abuse Mandated Reporter Training. Based on file review, staff have taken or renew Child Abuse Mandated Reporter Training.
-Staff files were incomplete. Based on file review, staff files are currently complete.
-Children's medication were expired. During today's visit, children's medication were checked. Per classroom teacher, one child no longer needs medication. One child is on vacation and parents have been notified to bring in new medication when child return to the program. Another child has brought in new valid medication.
The following Technical Violations issued on 12/12/2024 have since been corrected:
-Electrical wires were hanging. LPAs observed on this day dangling wires have been removed.
-One electrical outlet was not covered and wall plug was loose. LPAs observed a shelf has been placed to block the outlet.
-Cleaning solution has been removed from day care areas.
-Portable heater in one of the classrooms has been removed.
-Staff did not have proof of flu vaccine or Statement of Declining flu vaccine. During today's visit, staff's files have proof of flu vaccine or Statement of Declining flu vaccine.
-Children's files were missing some documentation. Director have since collected required paperwork from parents.
On this day, based on touring the center for Health and Safety Hazards and file review were conducted, LPAs cleared Type B deficiencies cited on 12/13/2024 and letter of clear deficiencies provided to Director.
A copy of this report and appeal rights were discussed and left with Director whose signature on this form confirm receipt of these reports. Notice of Site Visit was posted.
