| On March 13, 2026, Licensing Program Analyst (LPA) Leong arrived at the facility to conduct an unannounced Plan of Correction (POC) visit. This visit was a follow-up on previously cited deficiencies. LPA met with licensee Sumi Lee Levy and explained the purpose of the visit.
During today’s visit, the licensee and her assistant were supervising nine preschool-aged children.
LPA observed that the notice of the site visit on February 25, 2026, was posted on the bulletin board.
LPA verified that the following issues—previously observed on the date listed below—were not corrected during today’s visit:
- Mandated Reporter Training certificate: On February 25, 2026, LPA found that the licensee’s Mandated Reporter Training certificate had expired.
- Pediatric CPR and First Aid certificate- On February 25, 2026, LPA found that the licensee’s Pediatric CPR and First Aid certificate was not present in the home.
According to the licensee, she forgot to address these two requirements and will complete them as soon as possible.
A $400 civil penalty was assessed during today’s visit.
LPA reminded the licensee that failing to correct deficiencies by the POC deadline will result in a $100-per-day fine thereafter. The $100-per-day civil penalty will continue until the deficiencies are corrected.
A notice of site visit was given and must remain posted for 30 days.
