On April 16, 2026 at approximately 11:00am, Licensing Program Analyst (LPA) Quimbo conducted an unannounced, annual inspection. LPA met with licensee, Giovanna Luzardo, and explained the purpose of the visit.

Upon LPA's arrival, children were at the local park with 3 teaching staff. Present in the home was licensee and helper. LPA observed helper to have fingerprint clearance.

Hours of operation are Monday through Friday 9:00am to 5:00pm. Licensee operates their family child care home as a Spanish immersion program.

Facility is a multi-unit home. Licensee lives in their unit with their spouse and minor child. Per licensee, their adult child occasionally will visit the home. LPA observed adults living and/or working in the home have fingerprint clearance on file.

With licensee, LPA inspected home for health and safety hazards. The DAY ARE AREAS are the living room, dining room, kitchen (not currently in use and made inaccessible to children), bedroom #1 (not currently in use), sun room (classroom), lower level family room, bathroom and backyard. The OFF LIMIT AREAS are the rest of the home. There are stairs in the home that are made inaccessible with child safety gates.

LPA observed day care areas to be clean and organized. Children's toys and materials, when not in use, are stored in baskets that are labeled. Per licensee, toys and materials are sanitized daily. Furniture for children was observed to be age appropriate and in good condition. LPA observed electrical outlets to be made inaccessible with child safety covers. There is a fireplace in the home that is barricaded and made inaccessible to children. LPA observed large windows in main living room of home that have child safety locks installed.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Home is equipped with a fire extinguisher and a combination smoke and carbon monoxide detector. Detector was tested during today's visit and was observed to be in working condition.

Licensee provides a food service that includes lunch and afternoon snack. Per licensee, no children currently enrolled have allergies. Reusable utensils and plates are utilized for children's use and hand washed in home daily. If a child has a modified diet, an alternate food is provided to child. If children bring their own food to the home, food is stored in their individual lunch boxes that are labeled with their individual names. LPA observed knives to be stored in home's high shelf, inaccessible to children. LPA also observed cleaning solutions and poisons to be stored under a child safety locked cabinet. Kitchen was also observed to be made inaccessible to children with a child safety gate.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Bathroom for children's use was observed to be in working condition. Cabinet in bathroom also includes a child safety lock. There is a diaper changing table in the bathroom that was observed to be in good condition. Licensee provides diapers and wipes for enrolled children. Bathroom also includes a garbage bin that is disposed of daily.

Backyard is fully enclosed and fenced. LPA did not observe any pools, spas or bodies of water on site. Per licensee, outdoor toys and materials are brought out each time backyard is in use, cleaned after each use and stored inside the home when not in use. Backyard also includes an awning for shade, if needed.

At approximately 12:45pm, 12 preschool age children and 3 teaching staff arrived to the home. Licensee is operating within capacity limits and ratio during LPA's visit. Teaching staff present have fingerprint clearance on file. Per licensee, children spend majority of time in outdoor area of home.

Children sleep in the living room. Licensee provides sheets and blankets to preschool children that are laundered on site weekly. LPA observed children's sheets and blankets to be labeled with their individual names.

There are pack and play cribs available, if needed. LPA observed cribs to be free of loose articles and objects. Licensee has one infant enrolled, whom was not present during today's visit. Sleeping logs are maintained for enrolled infant as well as previously enrolled infants. LPA observed sleeping logs to document the date the infant is sleeping, the 15 minute time stamp of when the napping infant is checked on, the napping infants' sleeping position, and the initials of the individual checking on the infant. Sleeping logs from random months of January 2025, March 2025, August 2025, September 2025 and March 2026 were reviewed and maintained.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA reviewed facility records that included children's records, licensee's record, and teaching staff's records. LPA reviewed 6 random children's records that were observed to be complete. LIcensee's Pediatric CPR and First Aid certification is current and will expire 5/2028. Licensee's Mandated Reporter certification is also current and will expire 4/2028, that includes AB1207 certification. Licensee's required immunizations were also made available for LPA's review during today's visit.

LPA observed teaching staff present to also have current Pediatric CPR and First Aid certifications, current Mandated Reporter certifications and their required immunizations. Licensee maintains a childcare roster that was made available for LPA's review. Licensee also conducts and documents emergency disaster drills that was made available for review. Last emergency disaster drill was conducted 3/6/2026. LPA observed required licensing documents to also be posted in the home, available for review.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the Licensee Giovanna Luzardo, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
