At 9:50AM, LPA reviewed the backyard/outside area. The area was completely enclosed, with available playthings in good repair. The licensee has installed absorbent material (wood chips and padding) for added safety. The sandbox was observed free of potentially hazardous items. The home did not have pools, ponds, or other bodies of water on the premise.
During inspection, LPA reviewed facility records including the 4 children’s files and 2 personnel files. The staff files were reviewed and included the: Acknowledgement to Report Suspected Child Abuse (LIC9108).
LPA reminded licensee to ensure all staff’s proof of immunization and mandated reporter training course certificate is stored in the facility files. Advisory Note: Technical Violation: LIC9102 was issued.
The children’s files were reviewed and included the: Identification and Emergency Information (LIC700); Notification of Parent Rights (LIC995A); and Immunization Record.
At 12:15PM., Based on record review and interview, LPA confirmed facility is not documenting infant napping conditions after each 15 minute review. Advisory Note: Technical Violation (LIC9102TV) was issued.
The licensee, proof of current pediatric cardiopulmonary pulmonary resuscitation / first aid course was on file, and expires: 9/2027.
The licensee’s proof of the completed mandated reporter training course (AB1207) was on file, and expires: 9/2027.
Per licensee, facility conducts the required emergency disaster drills (fire and earthquake) every 6 months, with last drill done on, 4/30/2026, properly logged.
The required forms are posted in entry way and include the: Facility License Certificate; Notification of Parent’s Rights (PUB379); and the Emergency Disaster Plan (LIC610A).
Per license, isolation of an ill child is in the playroom. Per licensee, she provides daily snack/ meals service for the children. (REFER TO 809C, FOR CONT.)
