Licensing Program Analyst (LPA) Mok conducted an unannounced case management for a self-reported unusual incident dated 5/13/24. LPA met with the Senior Directors, Mary Tan, and Nicole Olsen, and they explained the purpose of the inspection to them. The facility submitted an unusual incident report about how staff handled a child inappropriately. Based on the collected documents and interviews that were gathered by the LPA, CCL couldn't get sufficient evidence to prove whether it happened or not at this point.

No deficiency was cited during the inspection.
