25 | On October 3, 2024 at approximately 12:45pm, LPAs Catrina Quimbo and Maria Olguin-Leon conducted an unannounced, plan of correction visit (POC) to Little Angels Nursery School. LPAs met with director, Carmen Villon, and licensee, Rabab Tawfik, and explained the purpose of the visit. Present during LPAs’ visit included 12 children and 4 staff.
On September 18, 2024, LPAs conducted an unannounced, case management visit at facility. During case management visit, at approximately 3:00pm, LPAs observed director to be in the facility with 5 children. LPAs observed director escort two children at separate times to the restroom (located in the back of the classroom). LPAs observed the remaining 4 children were unsupervised in the front of the classroom. At time of incident, no additional staff were inside the facility. Facility was issued a Type A citation and a civil penalty was assessed. A plan of correction and signed LIC9224s were discussed with director.
Plan of correction was for director to hold a staff meeting, reinforcing the importance of supervision. Facility was also issued a Type B citation for incomplete staff records.
As of this date, LPAs observed completed LIC9224s for 13 enrolled children. LPAs observed notice of site visit given during case management visit to be posted and available for review. Director provided proof of a staff training conducted on September 26, 2024. LPAs observed staff files reviewed during case management visit, to be complete on this date.
Two of the three deficiencies cited on September 18, 2024 have been cleared during today's visit. POC letters were provided. LPAs discussed remaining deficiency and POC with director.
Notice of site visit was given and must remain posted for 30 days.
