On 4/2/2024 at 8:30AM., Licensing Program Analyst (LPA), Luis Gomez met with Lead Teacher, Merve Perek. The purpose of today’s inspection was an unannounced, complaint inspection. This case management report is to cited for deficiencies found during inspection. Licensee, Rabab Tawfik arrived during inspection. Present was the lead teacher and 3 staff caring for 17 children. LPA inspected facility for health and safety hazards.
At 9:45AM., Based on record review, Interview and observations, LPA confirmed individual present (S1) without criminal record clearance or association on file. Per licensee, individual is present with children for interview and observation.
Amendment: deficiency and civil penalties dismissed.
At 11:00AM., Based on record review and interviews, LPA confirmed facility does not have a site director.
At 11:30AM., Based on record review, interview and observations, LPA confirmed facility fixtures, used for drinking water and food preparation, have not been tested for lead exceedance levels.
Based on today's inspection, deficiencies were observed in areas evaluated according to California Title 22, Div. 12 Chap. 1, Health and Safety, Code of Regulations and cited on 809D. Exit interview, Plan for Correction, and Report was reviewed with Licensee, Rabab Tawfik. Licensee’s signature of this form acknowledges the receipt of these documents.
