25 | On June 4, 2024 at 10:00am, Regional Manager (RM), Cindy Interiano, Licensing Program Manager (LPM) Marie Rodriguez, and Licensing Program Analyst (LPA) Catrina Quimbo met with licensee, Rabab Tawfik, for an informal office meeting. Licensee is the licensee for both 384004350 and 384002696.
The purpose of the informal meeting was to address licensee's non-compliance with facility.
Discussed during meeting included citations issued to facility 384002696:
- Child Care Center Director Qualifications: CCR 101215.1
- Drinking Water: CCR 101239.2.
- Personal Rights: CCR 101223.
- Written Directives-Lead Testing. CCR 101700.
- Food services. CCR 101227.
- Levy of Civil Penalty. HSC 1596.99
Licensee was advised of the following:
- Licensee cannot be the director for licensed child care center while licensed family child care home is in operation.
- A qualified director must be on site during operating hours.
- Kitchen must be made inaccessible to children.
- Drinking water cannot be withheld from children at any time.
- Food cannot be withheld from children at any time.
- CCLD recommends licensee change program's "restaurant style" food service policy to one of the following: "Family style" food service, obtain food vendor, enroll to food program, or children bring their own food to facility.
- Children must be supervised at all times.
- With a signed field form, public park can be utilized maximum use of once a week per group. Licensee must obtain a park permit from San Francisco Rec. and Park and an approved waiver from CCL.
- Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
