On 10/11/2023 at 1:20PM., Licensing Program Analyst, Luis J. Gomez met with Program Director, Rochelle Celedon. The purpose of the visit was explained and is for an unannounced; annual random inspection. Program is requesting an increase in capacity from 90 to 200. Program offers after-school care, which operates at St. Peter’s Catholic School. Program is utilizing eight classrooms: Parrish Hall: After-school Room, #4; Classroom #1 and #2, Middle School Room; Classroom #3, Main Building: Classrooms #101, #103, # 201, and 205. Program also has a shared, Library and Outdoor Play Yard. Days and hours of operation: Monday, Tuesday, Thursday, Friday: 3:00pm- 5:30pm and Wednesdays, 12:30- 5:30pm. Present were the directors and 14 staff supervising 86 children. LPA inspected the facility, indoor and outdoor, with director for health and safety hazards.

At 1:45PM., LPA observed the following: Classrooms were maintained clean and orderly. Classrooms inspected have books, supplies and materials available for the children. Classrooms were equipped with tables and chairs for activities.

At 2:00PM., Based on observations, LPA confirmed exposed heater vent in the middle school classroom (#3). Advisory Note: Technical Violation (LIC9102TV) was issued.

Classrooms have acceptable ventilation and lighting. Cubbies are available for storage of children’s belongings. Classrooms have functioning Carbon Monoxide Detector; Smoke Detector and fully charged fire extinguishers (2A10BC).

Due to program functioning at a school site, this school-age child care program is exempt from the square footage or toilet requirements.



At 2:20PM., LPA inspected the outdoor play areas and children’s bathrooms. Area was enclosed and free of trash or rubbish, with plaything in good repair. Water is available for the children with use of water station and disposable cups. Children’s bathroom located outside and were reviewed. Fixtures tested were in operating condition. Bathrooms had adequate supplies for handwashing. (REFER TO 809C, FOR CONT.)

At 3:00PM LPA review facility records including 5 children’s files and 8 personnel files. Staff files reviewed and included: Declaration to Report Suspected Child Abuse (LIC9108), and Notice of Employee Right (LIC9052).

At 4:20PM., Based on record review, LPA confirmed teacher's proof of qualifications missing from facility files.

LPA reminded director to ensure staff's updated ‘Mandated Reporter Training' (AB1207) certification, and proof of required immunization, stored in the facility files. Advisory Note: Technical Violation (LIC9102TV) was issued during inspection.

Children’s files were reviewed included: Identification of Emergency Information; and Notification of Parent’s Rights (LIC995).

Present staff members had their current CPR/ 1st aid certification on file.


LPA reminded facility to be conducting emergency disaster drills every six months, with last on done one on 5/24/2023.

Per director, food services are outsourced, and brought to facility daily.

At 4:30PM., Based on interview, observations, and record review, LPA confirmed facility's fixtures used for drinking water, and/ or food preparation have not been tested for lead contamination levels.

All required posting were reviewed including the facility included the License, Parents Rights (PUB393), Emergency Disaster Plan (LIC610). LPA reminded Director to post snack/ lunch menu in visible location for families, a least a week in advance. Advisory Note Technical Violation (LIC9102TV) was issued.

Director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPA referred director to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.

Prior to approval for increase in capacity, facility must:
1.Obtain Fire Clearance (STD 850).
2.Clear deficiencies cited during annual inspection.

Based on today’s inspection, deficiencies were observed in areas evaluated according to the Title 22 Division 12 of Ca. Code of Regulations and listed on the 809D pages. A copy of this report, exit interview, with the appeal rights was provided to Rochelle Celedon, and signature of this form acknowledges the receipt of these documents.



Civil Penalty of $250.00 was issued during inspection for repeat violation.

Notice of site visit was observed to be posted and shall remain posted for 30 days.
