On April 8, 2025 at approximately 1:00pm, Licensing Program Analyst (LPA) Catrina Quimbo conducted an unannounced, annual inspection. LPA met with head teacher, Ana Toba, and explained the purpose of the visit. Per head teacher, director is not present on this date due to sickness. Head teacher is designated with facility responsibility.

Present during LPA's visit included 22 preschool children and 4 teaching staff. Facility is operating within capacity limits and ratio during LPA's visit. All teaching staff present have fingerprint clearance on file. Preschool program operates a Spanish immersion Montessori program on the ground level of St. Aidan's Episcopal church. Hours of operation are Monday through Friday, 7:30am to 5:30pm.

With head teacher, LPA inspected day care areas, indoors and outdoors, for health and safety hazards. LPA observed a variety of Montessori toys, materials and equipment that are in good condition. Per head teacher, toys and materials are sanitized daily. Classroom includes areas for academic work, eating and rest and relaxation. There is age appropriate furniture that was observed to be in good condition. LPA observed a rug in the classroom that is clean, free of stains. Per head teacher, floor and rug are cleaned daily.

LPA observed electrical outlets in classrooms to be made inaccessible with child safety covers. Facility is equipped with a fire extinguisher, first aid kit, fire alarm system, fire sprinklers and carbon monoxide detector. LPA tested carbon monoxide detector during today's visit, which was observed to be working.

Chemicals, poisons, and cleaning solutions are stored in facility's high shelves, inaccessible to children. LPA observed cabinets and drawers in classroom to be free of poisonous materials. Storage for children's belongings are located inside the classroom, labeled with children's names and pictures. LPA observed a garbage bin with a tight fitting lid in classroom.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

There is a conventional oven in day care area. Per head teacher, children cook with oven (under staff's supervision) weekly. Per head teacher, when oven is in use, cones are placed around oven so children are aware not to go near oven.

Program provides a food service that includes morning snack and afternoon snack only. Children's families provide lunch for their children daily. LPA observed children's materials to be labeled with their individual names. Drinking water is made available through children's individual water bottles that can be refilled on site through kitchen sink.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care
Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Restroom for children's use was observed to be in working condition. LPA observed children's restroom to have age appropriate toileting equipment and hand washing materials. Per head teacher, children are required to be potty trained prior to enrollment in facility. There is a high cabinet in bathroom stored with poisons and materials that are made inaccessible to children.

Children sleep in the nap room. LPA observed children sleep on cots that are stored appropriately so that they are not hindering the entrance or exit to the room. LPA observed electrical outlets in napping room to be made inaccessible with child safety covers. Sheets are provided by the program while blankets are provided by families. Per head teacher, sheets and blankets are sent home weekly to be laundered while the cots are sanitized weekly as well.

Outdoor area is fully enclosed and fully fenced. There is a play structure that includes resilient padding underneath. Per head teacher, play structure is washed daily. Flooring in outdoor area is free of loose debris and hazards. LPA observed exit gates in outdoor area to have a bell hanging, if opened. LPA did not observe any pools, spas or bodies of water on site.

Program utilizes an electronic sign in/out record via an electronic application. LPA observed children present to be appropriately signed in during today's visit. Per head teacher, all individuals signing children in and out of program have codes that are specific to the individual only.

Licensing documents are properly posted and available for review. A child care roster is maintained and was also made available for review during visit. Program conducts emergency disaster drills that are documented and maintained. Last disaster drill conducted was 1/14/2025.

LPA reviewed 6 random children's records. Children's files include emergency information available for review. LPA reviewed files for all staff present during LPA's visit. All staff present have a current CPR/First Aid certification. Staff files also include a current Mandated Reporter certification. LPA reminded head teacher Mandated Reporter certification must be AB1207 certified and renewed by all staff every two years. LPA reminded head teacher all teaching staff must have a completed health screening report not more than one year prior to or seven days after employment. Technical violations were issued for incorrect certification and staff immunizations.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

No deficiencies were issued today, during LPA's visit. A notice of site visit was given and must remain posted for 30 days.
