On 4/24/2026 at 8:50AM., Licensing Program Analysts (LPA) Luis Gomez and Jovanna Badger, met with Teacher, Michelle Martinez. The purpose of today’s inspection was explained and was for an unannounced, 10-day complaint inspection. Present was the lead teacher supervising one child. The licensee, Nancy Azar and an addintional child arrived during inspection. LPAs inspection facility for health and safety hazards.

During today’s inspection, LPAs conducted interviews, reviewed records, and performed site observation.
During the course of this investigation, observations were conducted on: 2/19/2026; 4/13/2026; and 4/24/2026. The review of facility records was also complete, which included the staff files and children’s files. LPA conducted interviews with licensee and involved parties. (REFER TO LIC9099C, FOR CONT.)

Based on evidence collected, LPAs were unable to determine if rubbish was left in childcare areas. During inspection, LPAs observed facility trash/ rubbish was inside proper receptacles.
Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the complaint is Unsubstantiated.

LPAs conducted exit interview with licensee, Nancy Azar facility evaluation report (FER) was discussed. The Notice of Site Visit, and provider rights were be given.

Based on evidence collected, LPAs confirmed facility surfaces have excessive dirt and debris. During inspection, LPAs observed carpets, hallway, and furniture with particles and dirt build-up.

The preponderance of evidence standard has been met; therefore, the allegation is found to be SUBSTANTIATED. Title 22, California Code of Regulations (CCR, Title 22, Division 12, Chapter 1, Article 06).

Advisory Note: Technical Violation (LIC9102TV) was issued.


LPA conducted exit interview with licensee, Nancy Azar facility evaluation report (FER) and Technical Violation was discussed. The Notice of Site Visit, and provider rights will be given.
