On 6/23/2026 at 8:50AM., Licensing Program Analyst (LPA) Luis Gomez met with Teacher, Michelle Martinez. The purpose of today’s inspection was explained and was for an unannounced, complaint inspection. Present was the lead teacher supervising two children. The licensee/ director, Nancy Azar and 6 children arrived during inspection. LPA inspection facility for health and safety hazards.

During today’s inspection, LPA conducted interviews, reviewed records, and performed site observation. Based on information obtained during inspection, site director had an inappropriate conversation with staff in presence of day-care children. Advisory Note: Technical Violation was issued.

During the course of this investigation, on-site observation was conducted on: 4/24/2026 and 6/23/2026. The LPA reviewed facility records consisting of personnel files, children’s files, incident reports, and sign-in/ sign out logs. LPA conducted interviews with licensee, staff, children, and involved parties. (REFER TO LIC9099C, FOR CONT.)

Based on evidence collected, LPA was unable to determine if staff handled child in a rough manner. During interviews, staff indicated using appropriate intervention while assisting children.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the complaint is Unsubstantiated.

LPA conducted exit interview with Licensee/ Director, Nancy Azar and complaint investigation report was discussed. The Notice of Site Visit, and the provider rights were given.
