On Septmber 26, 2023 at approximately 3:30 PM, Licensing Program Analyst (LPA) Cowan conducted an unannounced, case management visit. LPA met with Executive Director, Sandra Hilliard, and explained the purpose of the visit. Present in the facility is directors, 50 staff, and 203 children in care.

The case management visit is regarding an unusual incident that occurred on September 15, 2023. Facility self reported incident to CCLD on September 20, 2023.

On September 15, 2023, in the morning, Staff was leaving the yard to make an emergency call to parents of another child. Just before leaving, C1 asked to go to the restroom. Staff took child to the restroom and made the necessary call. Staff forgot child was in the restroom and returned to yard. Incident report states that as soon as staff returned to the yard, staff remembered child in the bathroom. Staff returned to the bathroom to find child still there. Staff waited for child to finish in the restroom and they both returned to the yard.

Executive director states that child was alone for about one minute before staff returned to get child. This is a potential risk to children in care. A Type B citation is issued for this deficiency this day.


Exit interview conducted and report was and plan of correction reviewed with the executive director, Sandra Hilliard.

A notice of site visit was given and must remain posted for 30 days.
