On March 26, 2026 at approximately 8:35am, Licensing Program Analyst (LPA) Quimbo conducted an unannounced, annual inspection. LPA met with site supervisor, Vivan Lei, and site supervisor, Annabelle Tong, and explained the purpose of the visit.

Facility is licensed for a preschool and a toddler component. Main entrance to facility is located on Victoria Street. Preschool program currently operates in 6 classrooms: Grassland (2.5 years to 3 years old), Garden (2.5 years to 3 years old), Rainbow (3-4 year olds), Sunshine (3-4 year olds), Milky Way (4-5 years old), and Galaxy (4-5 year olds). Toddler component currently operates in 1 classroom: Seashell (18 months to 24 months).

Present during LPA's visit included 7 toddlers with 3 teaching staff and 77 preschool children with 14 teaching staff. Facility is operating within capacity limits and ratio during LPA's visit. Staff present have fingerprint clearance and are associated to facility number.

With licensee, LPA inspected entire facility for health and safety hazards. LPA observed licensing documents to be properly posted and made available for review in the main entrance of the facility. LPA observed licensing documents to include the facility's outdoor rotation waiver, sign in and out waiver, daily schedule, and menu. Required licensing documents (LIC forms) were observed to be posted, available for review.

LPA observed classrooms to be clean and organized. Children do not wear shoes inside the classroom. Classrooms have a variety of toys, materials and equipment that are in good condition and are age appropriate. Classrooms include areas for quiet rest, building blocks, manipulatives and writing. LPA observed toy bins to be labeled and numbered. Per licensee, toys and materials are sanitized daily and are deep cleaned quarterly. Flooring in classrooms include soft padding, rugs and hardwood floor that are free of stains and in good condition. Per licensee, floors are also cleaned daily and deep cleaned quarterly. LPA observed electrical outlets in classrooms to be made inaccessible with child safety covers. LPA also observed cleaning solutions to be made inaccessible to children in classrooms' high shelves.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

There are windows in the classrooms that have child safety locks. Storage for children's personal belongings are located both inside and outside the classrooms. LPA observed children's hooks and cubbies to labeled with their individual names and/or photos.

LPA observed classrooms to include compost bins with tight fitting lids. There are garbage bins for staff use that were observed to not have a tight fitting lid. LPA reminded licensee all garbage bins are to have tight fitting lids.

LPA observed each classroom to have an exclusive restroom or a shared restroom between the neighboring classroom. LPA observed restrooms to be in working condition. Accessible children's sinks were observed to not produce hot water. LPA observed restrooms to also be organized with children's personal clothing and diapering materials that were labeled with their individual names. Diapering children are changed in the bathroom that is within arm's reach to a sink. Diapers and wipes are provided by children's families and are appropriately stored. LPA observed diapers to be disposed of in individual garbage bags and placed in a garbage bin with a tight fitting lid.

Facility includes multiple fire extinguishers, a fire alarm, fire sprinklers and smoke detectors. LPA also observed facility to include a carbon monoxide detector. LPA tested carbon monoxide detector during today's visit which was observed to be in working condition. Classrooms also have emergency backpacks with emergency supplies, first aid kits, and emergency binders with children's emergency contact information.

Food service is provided and prepared on site in a kitchen. Food service includes breakfast, lunch and snacks. LPA observed food menu to be posted in facility. Per site supervisor, menu is also sent to families weekly. Kitchen was observed to be clean and organized. Reusable dishes and utensils are utilized for children's use that are washed and sanitized daily. LPA did not observe any utensils or food to be stored next to cleaning solutions. Kitchen includes multiple refrigerators that are separate for adult use and children's use.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16.

LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. LPA referred licensee to the Department website for lead: Lead Toxicity Prevention and Water Testing Information(https://www.cdss.ca.gov/inforesources/child-care-licensing/water-testing-information)

Children sleep in their designated classrooms on cots. LPA observed cots to be stored and stacked when not in use. LPA also observed cots to be labeled with children's individual names. Sheets and blankets are provided by children's families and are sent home weekly for laundering. LPA observed sheets to be stored and placed away appropriately when not in use, so that they are not touching each other.

There is one outdoor area that is shared between the preschool and toddler children. Outdoor area is located next to the facility. Children exit facility and walk to the outdoor area, supervised. Outdoor area is also used as a parking lot during morning drop off and afternoon pick up hours. When drop off and pick up hours are over, outdoor area is fully gated and enclosed. Per site supervisor, outdoor area gate closes at 9:00am every morning and reopens at 4:30pm, when children are no longer utilizing the outdoor area.

LPA observed outdoor area to include a variety of outdoor toys and materials that are in good condition. There is a climbing wall and play structure with rubber padding underneath. LPA observed sandbox to be covered when not in use. There are storage sheds in the outdoor area that store outdoor toys and materials. LPA did not observe any pools, spas or bodies of water on site. Drinking water is made available to children through their individual water bottles that can be refilled on site through filtered water systems. LPA observed children's water bottles to be labeled with their individual names during today's visit. There is also a multi-purpose room indoors, in the event weather does not permit outdoor use.

Program utilizes an electronic sign in and out application. Per licensee, each authorized individual has a unique code that is specific to the individual only. Specific code acts as the individual's signature. Sign in and out waiver was observed to be posted in the main entrance of facility. LPA also reviewed a random sign in/out sheet from a random day in March 2026 and reviewed today's sign in/out sheet. Sign in and out sheets are maintained and made available for review. Children are appropriately signed into program.

Program conducts emergency disaster drills at least once every six months which are documented and posted in each classroom. Last documented disaster drill was conducted March 13, 2026.

LPA reviewed 10 random, preschool children's records and 5 random toddler's records. Children's records were complete with physicians' reports, immunizations records, admission agreements and emergency contact information. LPA also observed toddler component children's records to have written authorization from the childs' authorized representative to participate in toddler component.

LPA also reviewed 12 random, teaching staff's records. Teaching records were complete with required immunization records, education transcripts and/or teaching permits, Mandated Reporter certificates and Pediatric CPR & FIrst AId certifications. Of the 12 staff's records, all staff were observed to have current CPR & First Aid certifications. LPA also confirmed toddler teaching staff have 3 units related to infant and toddler care.

Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
