13 | On July 1, 2024, Licensing Program Analyst (LPA) Van conducted an unannounced inspection to deliver the complaint findings in conjunction with unusual incident case management. The case management is documented in a separate report. LPA met with the new site Director, Amany Salah, and Executive Director, Luisa Baj. The purpose of the inspection was explained, and entry to the Center was granted. Today, 6 staff members supervised 12 children (4 infants and 8 toddlers.) The owner, Ashley Du arrived during the visit.
As part of the investigation, LPA interviewed teachers, examined medical records, and collected all relevant documents. Based on the information obtained, the child attempted to climb the single-side children’s books rack and accidentally fell. The teacher was in proximity, heard the child cry, and comforted the child. Based on the records reviewed, it was confirmed that the teacher-to-children ratio was met. This was verified through the children's sign-in and sign-out records, as well as the teachers' schedules and timecards.
Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. Therefore, the allegations are Unsubstantiated.
A notice of site visit was given and must remain posted for 30 days.
Exit interview was conducted, and the report was reviewed with the new Director, Amany Salah.
