13 | On April 03, 2024 at 1:45 PM, Licensing Program Analysts (LPAs), Tso and Mok made an unannounced complaint inspection and met with staff, Mei Hua Liang (Staff). Purpose of the inspection was explained. Present in the facility is 2 teachers, and 25 children. Children’s Roster and Sign in/out records were reviewed. During the inspection, observation, interviews, and records review were conducted.
Based on LPAs’ gathered information through observation, interviews and records reviews, the agency has investigated the complaint allegations above, overcapacity and over ratio. LPA Tso observed there were 8 children (4 is napping) in the preschool day care area supervised by 1 staff. There were 17 children in the classroom next to the preschool daycare area supervised by 1 staff. There are 25 children in total. But the facility license capacity is 23.
The facility failed to comply with the Staffing Ratio and Capacity regulations. The preponderance of evidence standard has been met. The above allegations are found to be SUBSTANTIATED. Two type A violations were issued today, April 03, 2024 in accordance with the California Code of Regulations, Title 22, Division 12 & Chapter 3, citations were being cited on the attached two LIC 9099Ds.
This report has been discussed and reviewed with the Staff whose signature confirmed have read the report. The Facility is advised to provide a copy of the Complaint Investigation Report and the Type "A" Deficiencies cited to the parents and guardians of children currently enrolled in care and parents of newly enrolled children during the next 12 months. A signed and dated LIC 9224 shall be maintained in all Children's files. This report will be maintained in the facility file and made available for public review three years after the thirty-day posting requirement has been met.

This report and a notice of site visit was given to the licensee and must remain posted for 30 days.
